A premium hand is one which is among the top possible hands. It can refer to a hand on any street, but more often refers to solely the starting hand. These hands are premium as they offer excellent odds.
There is no exact definition of what a premium hand is and it varies depending on an individual player's strategy. In hold'em everyone considers Sklansky group 1 to be premium holdings, whereas looser players may also consider Group 2 to be premium.
Terry is holding Q♣ Q♥ on the button and has raised Pasi, a loose player in mid-position. Pasi reraises pot. Terry has a premium hand, but knows that against a tight player he might not be in good shape. Pasi however is a kind of a maniac and is the type who thinks ATs is a premium hand. Thus Terry considers his chances very good at this point. He also knows that Pasi will make a large bet on the flop regardless of what comes, so he makes it simple and shoves.
